      This is a paid program that you may or may not have on your computer.

2.  Gmail users:

Sign in to your Gmail account using Google Chrome.  Search for “Google Docs Lyme Disease Brochure Template.”   At the top of the tool bar, click on “Use this template.”

Delete the text and pictures and replace with your own.  The form will save to your Google Docs/Drive within the Gmail account.

    Select a template.  Click on the pink box that says “Start Here!”  Need help?  Click on “Instructions.”   

    This can only be saved as a PDF file which cannot be edited.  This is a great template to use IF you can   

    start, finish, and print in one setting.
